Modificările testelor funcţionale respiratorii la pacienţii cu sarcoidoză endotoracică

Abstract

Lung functional disturbances in sarcoidosis are often described as restrictive, lately obstructive functional changes are increasingly found in patients with sarcoidosis, which are associated with a poor prognosis. A prospective, observational study was performed on 54 patients with intrathoracic sarcoidosis, followed up at Institute of Pulmonology and Tuberculosis "Cyril Draganiuc", Chisinau. Besides other usual investigations, patients underwent respiratory function tests, which were analyzed in our study. The aim of the study was to describe the lung function abnormalities and to determine the functional pattern frequently encountered in patients with pulmonary sarcoidosis. Results: Among all 54 patients, 8 (14.81%) subjects were detected with restriction, 13 (24.1%) with obstruction, and the remaining 33 (61.1%) - without pathological functional changes. Decrease in DLCO and MEF25 disorders were most common. In conclusion, the obstructive pattern in pulmonary sarcoidosis is frequently met, and these patients should be carefully followed up.

Modificările funcţionale din sarcoidoză sunt frecvent calificate ca fiind de tip restrictiv, în ultimul timp se pune accent tot mai mult pe modificările funcţionale de tip obstructiv, care la pacienţii cu sarcoidoză sunt asociate cu un prognostic nefavorabil. Studiul efectuat este de tip observaţional prospectiv, pe un lot de 54 pacienţi cu sarcoidoză endotoracică, evaluaţi în cadrul IFP „Chiril Draganiuc”, Chişinău. Au fost analizate rezultatele testelor funcţionale respiratorii. Scopul studiului este de a descrie tulburările funcţionale pulmonare şi de a determina pattern-ul funcţional mai frecvent întâlnit la pacienţii cu sarcoidoză pulmonară. Rezultate: Din numărul total de pacienţi, 8 (14,81%) au fost depistaţi cu restricţie, 13 pacienţi (24,1%) cu obstrucţie, şi restul 33 (61,1%) fără modificări funcţionale patologice. Scăderea DLCO şi MEF25 au fost tulburările cel mai frecvent întâlnite. În concluzie, pattern-ul obstructiv în sarcoidoza pulmonară este întâlnit frecvent, iar aceşti pacienţi necesită o abordare deosebită în evaluarea evoluţiei bolii.
