Abstract:
The complex treatment analysis of 149 of patients with acute destructive pancreatitis from 2006 to 2012 was made. In 72 (48.3%) of patients (the main group) along with the infusion therapy and operative treatment membrane plasmapheresis was performed. In the control group (77 (51.7%) of patients) endogenous intoxication syndrome correction was conducted without the extracorporeal detoxification. In the main group reduction of pain and improvement of general condition observed rapidly. By the end of the second day the level of amylase decreased by 53%. Due to membrane plasmapheresis, on the second day of treatment, decreasing of urea and creatinine levels by 62% and 56% compared with baseline values were detected. In the control group, any significant dynamics of amylase, urea, creatinine within 48 hours was observed. Decreasing of the number of necrotic suppurative complications in the main group 16.6% (in 12 of patients) compared with the control group - 32.4% (in 25 of patients). Mortality in the main group was 15.2%. In the control group the main causes of death were progressive septic condition, erosive bleeding, multiple organ and acute heart failure. Mortality was 27.2% (21 of patients).
