Background. Pulmonary hypertension (PH) is a common complication of chronic heart failure (cHF),
but its evolution in patients who are undergoing myocardial revascularization by coronary artery bypass
grafting (CABG) remains unclear. Objective of the study. Determining the evolution of PH, estimated
by echocardiography (EchoCG), in patients with different clinical phenotypes of HF after myocardial
revascularization by CABG. Material and Methods. Prospective analytical study, which included 126
patients (mean age 62.23±8.5 years, 67.5% were men) with cHF and CABG, divided according to HF
phenotype: HF with preserved ejection fraction (HFpEF), HFmid-range(mr)EF, HFreduced(r)EF,
investigated through EchoCG in the preoperative and early postoperative stage. Statistical analysis:
mean, frequencies, t-student test, ANOVA. Results. In the preoperative stage more than 50% of all
patients had a low probability of PH. The mean pulmonary artery systolic pressure (PSAP) was higher
in the HFrEF subgroup of patients (44.18±12.8 mmHg) in comparison with HFmrEF and HFpEF
subgroups (41.42±11.6 mmHg and 36.43±11.4 mmHg), p<0.05. Early after cardiac surgery there was a
reduction of mean value of PSAP in the general group of patients by 8.55±0.5mmHg, p<0.001 and the
rate of cases with low probability of PH increased by 28,5%, p < 0.05. The higher mean PSAP at this
stage (35.73±11.1 mmHg) was registered in the HFmrEF subgroup compared to those with HFrEF
(33.93±7.5 mmHg) and HFpEF (28.87±8.6mmHg), p<0.01. Conclusion. Both before and after heart
surgery EcoCG established a low probability of PH in most cases, with a higher percentage
postoperatively (79.8% vs. 51.3%). PSAP is superior in patients with HFrEF preoperatively, but
postoperatively it is higher in those with HFmrEF.
Introducere. Hipertensiunea pulmonară (HTP) este o complicație frecventă a insuficienței cardiace
cronice (ICC), însă rămâne neclar care este evoluția acesteia la pacienții ce suportă revascularizare
miocardică prin by-pass coronarian (BPc). Scopul lucrării. Cercetarea evoluției HTP estimate
ecocardiografic (EcoCG) la pacienții cu diferite fenotipuri clinice de insuficiență cardiacă după
revascularizare miocardică prin by-pass coronarian. Material și Metode. Studiu analitic prospectiv,
care a inclus 126 de pacienți (vârsta medie 62.23±8.5 ani, 67.5%- bărbați) cu ICC și BPc, divizați
conform fenotipului IC: IC cu fracție de ejecție păstrată (IC-FEp), IC-FEi - intermediară, IC-FEr -
redusă. Toți pacienții au fost investigați prin EchoCG preoperator și precoce postoperator. Analiza
statistică: media, frecvențe, test t-student, ANOVA. Rezultate. La etapa preoperatorie mai mult de 50%
dintre pacienți aveau o probabilitate mică de HTP. Valoarea medie a presiunii sistolice în artera
pulmonară (PSAP) a fost mai mare în subgrupul IC-FEr (44.18±12.8 mmHg) în comparație cu
subgrupurile IC-FEi și IC-FEp (41.42±11.6 mmHg și 36.43±11.4 mmHg), p<0.05. În perioada
postoperatorie precoce s-a observat o reducere a valorii medii PSAP în lotul general cu 8.55± 0.5mmHg,
p<0.001, iar rata cazurilor cu probabilitate mică de HTP s-a majorat cu 28,5%, p < 0.05. Postoperator
cea mai mare valoare medie a PSAP (35.73±11.1 mmHg) s-a înregistrat în subgrupul IC-FEi față de ICFEr (33.93±7.5 mmHg) și IC-FEp (28.87±8.6 mmHg), p<0.01. Concluzii. Atât preoperator, cât și
precoce postoperator, EcoCG a stabilit o probabilitate mică de HTP în majoritatea cazurilor, cu un
procentaj mai mare postoperator (79.8% vs 51.3%). PSAP este superioară la pacienții cu IC-FEr
preoperator, însă postoperator este mai mare la cei cu IC-FEi.
