Background. According to the ICD-10, psychogenic non-epileptic seizures (PNES) are classified as
dissociative disorders. Patients with drug-resistant epilepsy are at higher risk of developing PNES,
which are diagnosed in 20-30%. Objective of the study. Efectuarea diagnosticului diferențial între crize
epileptice și PNES-uri și demonstrarea combinației acestora în “diagnostic dual”. Material and
Methods. We present the clinical case of a 42-year-old patient, diagnosed with epilepsy from the age of
13, with focal seizures, with altered awareness and evolution to bilateral tonic, suspected of being drugresistant on polytherapy with Carbamazepine 1200 mg/day, Levetiracetam 3000 mg/day and Topiramate
50 mg/day. Results. The patient was investigated by brain magnetic resonance imaging - right frontobasal posttraumatic sequelae, video-EEG monitoring with recording of 2 paroxysmal events: one
without electrographic correlation, suspected to be PNES, and other with electrographic correlation -
epileptic seizure with frontal semiology. The patient was assessed by a psychologist and psychiatrist
with the identification of intra-family conflicts, established moderate global cognitive deficits, anxiety
and severe depression. Subsequently the scheme of antiepileptic treatment was modified,
antidepressants in combination with psychotherapy sessions was added. Conclusion. In the management
of drug-resistant epilepsy it is necessary to consider the possibility of epileptic seizures and PNES. Also,
it is important the electrographic correlations on EEG and multidisciplinary team collaboration.
Introducere. Conform ICD-10, crizele psihogene non-epileptice (PNES) sunt clasificate ca tulburări
disociative. Pacienții cu epilepsie farmacorezistentă manifestă risc crescut în dezvoltarea PNES-urilor,
fiind diagnosticate în 20-30%. Scopul lucrării. Pentru a face un diagnostic diferențial între crizele
epileptice și PNES și pentru a demonstra combinația celor în „diagnosticul dual”. Material și Metode.
Se prezintă cazul clinic al unei paciente de 42 de ani, diagnosticată cu epilepsie de la 13 ani, cu crize
focale, alterarea conștienței și bilateralizare tonice, suspectă a fi farmacorezistentă pe politerapia cu
Carbamazepină 1200 mg/zi, Levetiracetam 3000 mg/zi și Topiramat 50 mg/zi. Rezultate. Pacienta a
fost investigată prin rezonanță magnetică cerebrală – sechele posttraumatice fronto-bazale pe dreapta,
video-EEG monitorizare cu înregistrarea a 2 evenimente paroxistice: unul fără corelare electrografică,
suspect a fi PNES și celălalt cu corelare electrografică – criză epileptică cu semiologie frontală. A fost
evaluată de psiholog și psihiatru cu identificarea de conflicte intrafamiliale, stabilite deficite cognitive
globale moderate, anxietate și depresie severă. Ulterior, schema de tratament antiepileptic a fost
modificată, cu asocierea de antidepresive și cu ședințe de psihoterapie. Concluzii. În managementul
epilepsiei farmacorezistente se impune a lua în considerare posibilitatea prezenței, pe lângă crizele
epileptice și a PNES-urilor, importanța corelărilor electrografice la EEG și a colaborării în echipă
multidisciplinară.
