Abstract:
Introduction : The prognosis for life in patients with locally advanced malignant
tumors is extremely poor in any primary location. The invasion of adjacent organs
and trunk vessels over the decades has been considered unresectable for technical
reasons and also for oncological radicalism. The aggressive surgical approach will
extend the limits of the resection and in the association of perioperative
chemotherapy can provide greater survival in this patient population. Material and methods: We analyzed 54 patients with locally advanced
malignant tumors with invasion in neighboring organs, which underwent
multivisceral resections. From the total group 12 patients were
diagnosed with gastric cancer, 24 patients with colorectal cancer, 18
patients with pancreatic cancer. Results: R0 resection was achieved in 86% of cases. Postoperative morbidity 7.4%, morbidity
21.4% and are directly proportional to the number of anastamosis performed in stages of
reconstruction. Conclusions: The morbidity and mortality of multiorganic
resections is significantly higher than standard resections,
therefore requires careful selection of patients for aggressive
surgical approach. Multivisceral resections have curative
potential and are feasible in R0. Overall survival in locally
advanced in IV stage for patients after multivisceral resection
is much longer than patients after palliative treatment.
